Course Fees
The total cost for the course is $400 plus $100 materials fee for a total of $500. All course materials are included except textbooks. Textbooks are ordered separately. There are no hidden charges.
The course textbooks can be ordered online through the MBS Direct website, or from any retail or textbook supplier you choose.
Enrollment may be terminated by the University of North Dakota Office of Certificate Programs if you do not make satisfactory progress.
Please indicate on your application if you prefer the materials in print form or on CD-ROM.
Application
Click on the course application link below or in the menu bar, print out and fill in the application (or fill in the application in Acrobat Reader and print it out), obtain the required signatures and submit the application to our office by mail or fax with your payment and a copy of your dietitian's CDR card. The signatures on the application indicate agreement to the procedures and requirements presented in the brochure.
